PHYSICIAN: MD / DO
FULL TIME
FORT COLLINS, CO

We’re looking for physicians who are passionate about caring for patients and their families and are excited about doing this in an innovative way. Our physicians will be responsible for assessing medium to high acuity patients in their homes. They will need to be flexible, autonomous, and be able to problem solve effectively. They will partner with our team of nurse practitioners, nurses, paramedics, social workers and other ancillary staff to help us deliver acute hospital care to the home and bring patients to better health.

Responsibilities

  • Admit patients to Origin Healthcare
    • Determine appropriateness of “hospital at home” service and accept candidates to the service when care can be provided efficiently and safely at home.
    • See patients in person at their home or via telehealth visit within 60 minutes depending on acuity and staffing.
    • Review admission treatment plans with a multidisciplinary team
  • Daily rounding visits to patients via telemedicine and occasionally in person, in their homes, with daily adjustment of treatment plan(s) as needed for that individual.
  • Review of patients and treatment plans with a multidisciplinary team, including nurse practitioners, nurses, paramedics, and ancillary support staff as needed.
  • Telemedicine visits when needed for urgent assessment and triage.
  • Coordination of care between all members of the Origin Healthcare team as well as outpatient providers.
  • Assessment of any social determinants of health needs and development of a plan, with ancillary team support, for potential helpful interventions.
  • Conduct follow up calls to ensure patients are continuing to improve and have good outpatient support.
  • Know when a patient would benefit from a higher level of care and coordinate this care as needed
  • This role may require some night, weekend, and holiday coverage.

Qualifications

  • At least 5 years of experience post residency
  • At least 3 years of experience in a hospital setting
  • Current board Certification in Internal Medicine, Hospital Medicine, or Family Medicine
  • Be in good standing with your medical board and have a current and unrestricted license to practice.
  • Ability and desire to practice medicine with deep knowledge, respect, and empathy
  • Excellent written and verbal communication skills with patients and other clinicians
  • Passionate about continuous education and improvement
  • Proficiency and comfort with technology; experience with telehealth a plus
  • Flexibility and comfort working in a rapidly-changing, new environment
  • Valid driver’s license and reliable transportation

We are interested in partnering with physicians who are passionate about caring for patients, supporting families and who are excited for the opportunity to deliver care in a safe and innovative way. Our physicians are responsible for providing and overseeing the care of medium to high acuity patients in their homes.  Our physicians lead teams, exercise exceptional clinical judgment and are responsive to situational needs. Through a collegial partnership, our team of physicians, NPs, nurses, paramedics, social workers, and other ancillary staff deliver acute hospital care to the home and bring our patients to better health.

Responsibilities

    • Admit patients to Origin Healthcare

      • Determine patient eligibility of “hospital at home” service and accept candidates when care can be provided efficiently and safely at home as per Origin Healthcare standards and policies.

      • Evaluate patients in person via telehealth visit in conjunction with in-person visit by NP and RN within 60 minutes of admission.

      • Implement a customized admission treatment plan with the assistance of a multidisciplinary team

    • Daily rounding visits to patients via telemedicine with daily adjustment of treatment plan(s) as needed

    • Review of patients and treatment plans with a multidisciplinary team, including nurse practitioners, nurses, paramedics, and ancillary support staff as needed.

    • Telemedicine visits when needed for urgent assessment and triage.

    • Manage triage calls from patients, family and other providers. Make appropriate recommendations and ensure appropriate follow up scheduled if needed.

    • Coordination of care between patient, family and all members of the Origin Healthcare team as well as outpatient providers.

    • Assess for social determinants of health needs. In conjunction with social workers, develop treatment plans and make appropriate referrals to fill gaps in care identified.

    • Provide direct communication with the primary care team at time of discharge to ensure smooth transitions of care.

    • Have high clinical awareness of when a patient requires reassessment or would benefit from a higher level of care. Facilitate this care as needed.

    • Assess the patient and family’s understanding of their illness, determine any barriers to health and provide necessary resources, referrals and education.

    • Thoroughly document clinical events and patient care plans. Accurate billing and coding of encounters.

    • Actively participate in all company quality and safety programs.

    • Maintain regulatory compliance with all Origin Healthcare and other regulatory entity requirements.

    • This role may require some night, weekend, and holiday coverage.

 

Qualifications

We’re looking for nurse practitioners who are passionate about caring for patients and their families and are excited about doing this in an innovative way. Our NPs will be responsible for assessing medium to high acuity patients in their homes. They will need to be flexible, autonomous, and be able to problem solve effectively. They will partner with our team of physicians, nurses, paramedics, social workers and other ancillary staff to help us deliver acute hospital care to the home and bring patients to better health.

Responsibilities

  • Clinically assess patients in their homes within 60 minutes of admission and create a treatment plan, in collaboration with a physician, based on the patient’s needs.
  • Daily visits to patients in their homes and adjustment of treatment plan(s) as needed for that individual.
  • Daily review of patients and treatment plans with a multidisciplinary team, including physician, nurse, and other ancillary staff.
  • Occasional telemedicine visits when needed for urgent assessment and triage.
  • Coordination of care between all members of the Origin Healthcare team as well as outpatient providers.
  • Assessment of any social determinants of health needs and development of a plan, with ancillary team support, for potential helpful interventions.
  • Conduct follow up calls to ensure patients are continuing to improve and have good outpatient support.

  • Must utilize critical thinking and nursing experience to assess patients with a variety of clinical and social conditions.
  • Effectively communicate with members of the interdisciplinary care team and with patients and families.
  • Intervene and provide professional nursing treatments in the patient’s home under general medical supervision.
  • Accurately document clinical events and patient’s care plans.
  • Maintain a high degree of racial, cultural and social sensitivity.
  • Assess the patient and family’s understanding of their illness, any barriers to health and be able to provide resources and education.
  • Job does require commuting and applicants must be comfortable working within a patient’s home.
  • This role may require some night, weekend, and holiday coverage.

Qualifications

  • BSN or higher from an accredited school of nursing, diploma must be provided upon hire
  • Current and active Colorado R.N. License
  • BLS certification
  • ACLS certification, required within 6 months of hire
  • Previous experience as an RN in an acute care setting, preferred

We are interested in partnering with paramedics who are passionate about caring for patients, supporting families and who are excited for the opportunity to deliver care in a safe and innovative way. Our paramedics exercise exceptional clinical judgment and are responsive to situational needs. They are responsible for assessing medium to high acuity patients in their homes and coordinating care with a physician or nurse practitioner to determine best next steps for care, this may include symptom management and treatment in the home or may require coordination of hospital admission..

Responsibilities

  • Clinically assess patients in their homes who are experiencing an acute health concern.
  • Utilize critical thinking to assess nature and extent of illness and to triage care
  • Provide care and treatment in accordance with physicians orders and standards of practice
  • Effectively communicate with the interdisciplinary care team as well as with patients and families.
  • Accurately document clinical events and resulting treatment plan.
  • Participate in multi-disciplinary patient review meetings with other team members as needed.

Qualifications

  •  Colorado EMT- P level certification
  • BLS certification
  • ACLS certification
  • High school diploma required, Bachelor’s degree preferred.
  • 1+ years of previous experience as a community paramedic preferred
  • CP-C required within one year of hire
